A Social Prescriber/ Wellbeing Coordinator is someone that helps support individuals to empower them to take back control and be able to enjoy a better quality of life.
This is achieved with the role being focused on addressing ‘What matters’ to the individual and working with them to produce person centered goals.
The Health & Wellbeing Team plays a key role in the Primary Care Network working with Combe Coastal Practice (Ilfracombe, Woolacombe & Combe Martin)

Your Voice, Your Future, Your Community!
Are you 16 – 24 years old? Do you live in North Devon or Torridge? Do you want to earn up to £375?
Kailo is a research and design programme that has spent 4 years listening to, and working with, young people to understand what needs to change to better support young people into employment.
What’s the commitment?
Around 12 two hours sessions from July/August – October, held in Northern Devon.
