At One Ilfracombe, we’re committed to improving health, boosting economic prosperity, and creating a high-quality living environment for our residents. Our Living Well initiative focuses on identifying health and wellbeing challenges in the community and working collaboratively to address them. By listening to local people and those who provide support services, we develop projects that fill gaps in care and offer help where it’s needed most.
Our Living Well team brings together a wide range of people and organisations dedicated to the wellbeing of Ilfracombe residents. This includes GPs, dentists, wellbeing practitioners, business owners, community nurses, housing officers, local churches, schools, adult education providers, the Salvation Army, Action for Children, the food bank, and volunteers. We also work with support services covering mental health, debt and financial advice, addiction, homelessness, energy usage, older people’s services, dementia, family conflict, disability, carers, and equality and diversity.
Every month, our team meets to share knowledge, discuss community issues, and collaborate on solutions.
We’ve also partnered with the NHS to deliver important health programmes, including diabetes awareness, cardiac prevention, and Home from Hospital support.
As pioneers of social prescribing in the UK, One Ilfracombe continues to make a difference. Through our partnership with the local Primary Care Network, we employ a Wellbeing Team based in local GP surgeries. Their role is to support patients with their social needs, helping them access the right resources and services to improve their overall wellbeing.
